RESUMO

Objective: To determine, through a systematic review, the effects of halo gravity traction in spinal deformity. Methods: Prospective studies or case series of patients with scoliosis or kyphosis treated with cranial halo gravity traction (HGT) were included. Radiological outcomes were measured in the sagittal and/or coronal planes. Pulmonary function was also assessed. Perioperative complications were also collected. Results: Thirteen studies were included. Congenital etiology was the most frequent etiology observed. Most studies provided clinically relevant curve correction values in the sagittal and coronal planes. Pulmonary values improved significantly after the use of HGT. Finally, there were a pool of 83 complications in 356 patients (23.3%). The most frequent complications were screw infection (38 cases). Conclusions: Preoperative HGT appears to be a safe and effective intervention for deformity that allows correction prior to surgery. However, there is a lack of homogeneity in the published studies.(AU)


Objetivo: Determinar, mediante una revisión sistemática, los efectos de la tracción de halo-gravedad (HGT) en las deformidades de columna. Métodos: Se incluyeron estudios prospectivos o series de casos de pacientes con escoliosis o cifosis tratados con HGT. Los resultados radiológicos se midieron en los planos sagital y/o coronal. También se evaluó la función pulmonar. Finalmente, se recogieron las complicaciones perioperatorias. Resultados: Se incluyeron 13 estudios. La etiología congénita fue la más frecuente. La mayoría de los estudios proporcionaron valores de corrección de la curva clínicamente relevantes en los planos sagital y coronal. Los valores pulmonares mejoraron significativamente tras el uso de HGT. Por último, existieron 83 complicaciones en 356 pacientes, siendo la infección la más frecuente (38 casos). Conclusiones: La HGT mostró ser una intervención segura y eficaz para la deformidad, que permite la corrección antes de la cirugía. Sin embargo, existe una falta de homogeneidad en los estudios publicados.(AU)

Objetivo: Se realizó un metaanálisis para evaluar los resultados clínico-funcionales de la cirugía de ligamento cruzado anterior en militares, así como las complicaciones asociadas a la misma. Material y métodos: Se realizó una búsqueda en las 3 principales fuentes de bases de datos hasta diciembre de 2022 (PubMed, Google Scholar y ScienceDirect) en cuanto a los resultados tras la cirugía de ligamento cruzado anterior en personal militar. La revisión sistemática se realizó siguiendo las normas Preferred Reporting Items for Systematic Reviews and Meta-Analyses –PRISMA–, y los criterios de inclusión siguieron la estrategia PICO. Los datos de los estudios incluidos se analizaron mediante el software Review Manager 5.4. Resultados: Se seleccionaron un total de 7 estudios retrospectivos. La tasa general de retorno a la actividad completa en el personal militar fue del 62,3% (61,5% para el grupo de no oficiales frente a 68,3% para el grupo de oficiales), sin ser esta diferencia significativa (p=0,92). La tasa general de lesión meniscal en los militares fue del 58,8%, sin ser esta diferencia significativa (p=0,88). La homogeneidad en ambos casos fue buena (I2=0%, p=0,99). Conclusión: El regreso a la actividad militar completa puede usarse en la población militar como un marcador de éxito después de una reconstrucción de ligamento cruzado anterior. Hay que destacar que una gran cantidad de militares experimentan limitaciones permanentes en la actividad que impiden el regreso completo al servicio.(AU)

OBJECTIVE: To compare short versus long intramedullary nails for intertrochanteric hip fractures in terms of efficacy and safety. METHODS: We included cohort studies and randomized clinical trials. The methodological quality of the studies was assessed by the Newcastle-Ottawa Scale. The meta-analysis was performed using the Review Manager 5.4. Heterogeneity was checked with the I2 test. A fixed effects model was adopted if there was no heterogeneity. RESULTS: Twelve studies were included. The reoperations rate was lower in the short nail group (OR: 0.58, 95%CI: 0.38-0.88) and there were no differences regarding the peri-implant fracture rate (OR: 1.77, 95%CI: 0.68-4.60). Surgery time and blood loss was significantly higher in the long nail group (MD: -12.44, 95%CI: -14.60 -10.28) (MD: -19.36, 95%CI: -27.24 -11.48). There were no differences in functional outcomes. CONCLUSIONS: The long nail showed a higher reoperation rate, blood loss and surgery time compared to the short nail. LEVEL OF EVIDENCE: III.

INTRODUCTION: femoral lengthening using an intramedullary nail is one of the surgical options in the treatment of severe lower limb dysmetria in routine clinical practice. MATERIAL AND METHODS: a retrospective descriptive study was carried out on a series of five patients with a mean age of 15.4 years, who underwent femoral lengthening surgery using a Precice® intramedullary nail. The etiology in all cases was idiopathic. Preoperative and definitive postoperative theoretical lengthening or dysmetry was measured, as well as lengthening accuracy, distraction rate and index (mm/day and days/cm, respectively) and consolidation index (days/cm). Intraoperative and postoperative complications were identified in all cases. RESULTS: mean follow-up was 21 months (12-42), with no loss to follow-up. The mean duration of the surgical procedure was 126 minutes (105-160). The preoperative theoretical dysmetry was 38 ± 2.7 mm. The final mean lengthening was 41 ± 7.5 mm. The mean accuracy was 108% (91-125) and the distraction rate was 0.9 ± 0.4 mm/day. The distraction rate was 13.9 ± 5.1 days/cm and the consolidation rate was 26.6 ± 9.1 days/cm. Bone consolidation was observed in all patients with a mean of 113 ± 58 days. Regarding complications, a total of four minor muscular complications were found. CONCLUSION: the Precice® intramedullary nail is a good treatment option for cases of severe femoral shortening, providing good clinical and radiological results with a low rate of complications and implant failure.


INTRODUCCIÓN: el alargamiento femoral mediante clavo intramedular es una de las opciones quirúrgicas en el tratamiento de las dismetrías severas de miembros inferiores en la práctica clínica habitual. Objectives: To determine the incidence of community-acquired pneumonia and describe its characteristics. To assess differences influenced by Fine’s pneumonia severity index. Methods: Prospective, descriptive study of patients with community-acquired pneumonia treated over a period of1 year in the emergency department of Hospital General Universitario in Alicante, Spain. Social, demographic and clinical variables (including laboratory, radiologic, and microbiologic data) were collected. Destination on discharge from the emergency department and patient status at 30 days were recorded. The pneumonia severity index was determined according to Fine’s prediction rule, and patients were then classified as being at low (< III) or high (> III) risk. Differences between the 2 risk classes and the distribution of admissions according to risk were analyzed. Results: Five hundred fifty patients with community-acquired pneumonia were included. The cumulative incidence was2.2 cases per 1000 patient-years. Patients with community-acquired pneumonia at high risk had more comorbidity and functional decline, a higher incidence of respiratory failure, and infiltrates in multiple lobes. An etiologic diagnosis was established for 209 patients (38%). The most common microorganism isolated was Streptococcus pneumoniae in all risk classes. The admission rate was 77.2% (high-risk classes, 99.5%; low-risk, 65.1%). The patients were admitted to the respiratory medicine department, the short-stay unit, and the internal medicine department. Risk class influenced patient destination on discharge from the emergency department (AU)


Objetivo: Conocer la incidencia y características clínicas de los pacientes con neumonía adquirida en la comunidad (NAC) y reflejar las diferencias en función de la gravedad determinada por el índice de Fine (IF).Método: Estudio descriptivo y prospectivo de los pacientes con NAC atendidos en el servicio urgencias (SU) del Hospital General Universitario de Alicante durante un año. Se recogieron variables sociodemográficas, clínicas, analíticas, radiológicas, microbiológicas y relacionadas con el destino al alta. Se realizó seguimiento a los 30 días. Se determinó la gravedad de la NAC según el IF, y se clasificó en NAC de bajo riesgo (..) (AU)

RESUMO

Guava (Psidium guajava L.) is a tropical fruit widely relished in the tropics, consumed fresh and processed. In this work, free acidity, pH, ash, nitrogen and water contents were measured, besides the total polyphenol content and the antioxidant capacity of the peel, the shell and the pulp of the fresh fruit and the processed guava pulp and jam. The highest phenolic content was found in the guava skin la (10.36 g/100 g skin) and the lowest in the jam (1.47 g/ 100 g jam), in dry weight. The antioxidant capacity of the skin was 10 times higher than that of the pulp, and the jam was twice that of the shell.

RESUMO

La guayaba (Psidium guajava L.) es una fruta tropical de gran aceptación en los trópicos, donde se consume fresca y procesada. En este trabajo se comparó la acidez libre, el pH, el contenido de cenizas, nitrógeno y la humedad, junto con el contenido de polifenoles totales y la capacidad antioxidante de la piel, el casco y la pulpa de la fruta fresca, y de la pulpa procesada y la mermelada de guayaba. El mayor contenido de polifenoles fue encontrado para la piel de la guayaba (10.36 g/100 g piel) y el menor en la mermelada (1.47 g/100g mermelada), expresados en base seca. Se encontró que la capacidad antioxidante de la piel fue diez veces superior a la de la pulpa, y la de la mermelada el doble que la del casco
